I usually wear a soft sweatshirt (60%cotton / 40% polyester) when I practice. When I'm finished I wipe down the guitar with the sleeve. I guess I'm not very dirty, so, the guitar does not get very dirty, and I only wear the sweatshirt for a few days before washing it.

Always after playing. Most importantly for me, the strings and fretboard. But in general, the entire guitar gets a good wipe and during that time I give it a quick once over for potential issues.
