I play seated, with a strap. I have the guitar central to my body and I have the head stock raised to facilitate easy access to the first frets. If I need more leverage for barres I can adjust the head stock up as needed prior to playing the tune.

I highly suggest a neck up type of strap or a support. You will surely find a position that works for you.
