A reason has to exist as to why a truss rod nut can wind down to the root section of the threads, I find this reason occurs from plastic deformation of the rod over a period of time, when you add washers to extend the nut back into a threaded area and apply tension, then further deformation occurs to the point of failure, typically it necks in the root area of the threads

Ah got ya, going after the root cause is always a good idea.

You mean if you unscrew it just a bit you can't even screw it back to where it was?
