Seldom.
I used to lose lots of picks, and sometimes lose slides. I switched to playing with my fingers and haven't played slide for ages: now I know exactly where all my picks and slides are, and I don't care because I don't need them.

When lose something it's a pick. It's been a few years since I have lost one.
I quit carrying a pick unless I know I will be someplace where I might need one.

I moved a couple of years ago and was trying to find one of my pedals. I checked and rechecked all my packed/unpacked boxes and suitcases. I found the empty box but not the pedal. Then I realized that I hadn't lost a pedal - the store didn't have the proper box for the pedal so they used the box of another pedal in that line. I thought I had two of that same pedal but only one.

My trick is to have so many of each (capos, picks, tuners, cables) that I have no idea what I have, so I never miss anything.
