What process do you use to design these? Are you making drawings first? Do you work directly with the inlay through trial and error?

Sorry I didn't see your question earlier. I always create the drawings first. It's MUCH easier to change shapes on paper than it is with shell. I like to start with a theme and then build around that idea. I generally like to collect images and then combine them in Photoshop. Once I have something I like I create a line drawing which serves as my cut pattern.
