Default Adjusting the truss rod - strings loose or not?

Should the strings be loose when adjusting the truss rod on the guitar or is it ok if I do this adjustment while tuned in standard?

Standard tuning. Don't force it (let a luthier deal with a sticky truss rod). 1/8th of a turn or less at a time. Take your time between adjustments. Re-check 24 hours later to see if the neck moved further after you stopped.

Agreed re leaving it in tune,

but..

if it is a 12 string or bass guitar, I go more gently, dependant on how the truss rod feels when I go to adjust it, I may detune before going any further if it feels heavy and tight.

Have repaired many broken truss rods, so knowing how much work is involved, I err on the safe side myself.

It's safer to detune, but not necessary if the truss rod is in good condition. I usually use blocks and clamps to flex the neck into the desired relief, then tighten the rod to hold it at that relief.
As Steve says, replacing a truss rod is not a trivial repair, and some designs are more robust than others, so I always err on the side of caution.
