Music theory is like chemistry. There are as many exceptions as there are rules.
F# is the third. It is not the 11th. Like chemistry, music theory is not whatever you want it to be.

let the bass player catch the F# while playing the D chord with the (G) suspended. Solves all the finger stretching issues. With so many 3 chord songs the (slash) chords often refer to suggested bass movement by the (bass or piano) player

No bass player. Just me, mostly for my own entertainment. I'm now playing a regular D, adding my pinky on the third fret of the high e, as I think you are suggesting. It's pretty easy to hit because I'm used to playing a D and the pinky is already there. I'm working on the thumb, but it sounds ok just hitting the top five strings, so I can add the thumb when I'm ready.

Or C, G, D, which is how Zevon plays it in the live videos I've seen. One reason I like this version is the way you can play the opening riff by pulling off and hammering on from C to G so cleanly. The upstrokes on the Gs in that repeating riff just sound so right. What can I say? It's a challenge working it out, but that's part of the fun.

It's a great song. I play it using C/G, G and D (plain C during the bridge). Not saying it's the greatest, but it works!
