If the problem is the later, try rolling your index finger a little to the left (toward the nut) so you are using the less fleshy part of the finger to press the strings, rather than the soft flesh that runs down the center of your finger.

It may just be down to practice at this point.... my transitions are just plain nasty. Once I get there, the chord is pretty clean.... just takes do **** long to get there!
