Default Wiring Help - 1/4" and XLR input

Hey guys,

Having a bit of trouble with a friend's guitar. It's a Washburn with a Preamp EQ and both a 1/4" jack and XLR connection. When I got the guitar the wiring was mangled to the point where I couldn't tell what went where. After stripping thing back I have a 5 wire lead from the Preamp/EQ; red, white, blue, black, and bare wire. The 1/4" jack is a three prong, the XLR has 4. I've hunted all over the net for some kind of diagram or picture example but I'm stuck. If any of you have experience in with these things any help would be greatly appreciated.
