Bryan,

It's called an EMG Ultrajack. NOT cheap. I can't remember just now where I got the last batch, but I chase down a dealer and buy enough that they give me a discount.

EMG and Switchcraft both have a handle on making a reliable connection with 1/4" hardware. Everyone else not so much.

It is an active pre-amp with UST and condenser mic.
