I believe that it was an exact tab and sheet music from John Fahey's Christmas CD. I got my copy years ago via E-bay.

It's probably long out of print now. There appears to be a copy of it available on Amazon for $80

I wish this book was available at a reasonable price. A PDF was available on the Internet at one time but was taken down. I would gladly pay a non-scalper price for it.
