It's tab that I created using an Excel spreadsheet. I set up different spreadsheets for 4-4, 3-4, 12-8 time. For example, the 4-4 spreadsheet has eight columns per measure for 8 eighth notes. The six rows just represent the six strings. It's a little clunky, but it works for me. I would be happy to email the blank spreadsheets to anyone who is interested.
