Yes it did, there are some issues but I would suggest what Kerbie said. Gruhn does a fabulous job with added pictures sent to them, that is if you don't live near Nashville.

yes! Cool guitar. Dude I went to high school ('73) played one. Loved it!

I had a Yamaha.

That link works and does appear to be a 12 fret 000-45, although I do not claim to be an expert in prewar Style 45 Martins. I do know some people who could.
