I almost never use a flat pick (Probably 98% fingerstyle - even on the Strat).

But, when I do, I use the technique others have mentioned, where they center the pick on the (inside of the) back of the guitar )opposite the sound hole), then quickly flip the guitar 180 degrees, keeping it level from headstock to butt, and the pick almost invariably falls out, or ends up right up against the strings. A bit more shaking and the pick is usually free.

If I had Mancuso's jaw-dropping fingerstyle technique, I'd do away with using a flat pick forever; but that's probably just a pipe dream :|.

I have trained a ferret to retrieve picks, but I can't recommend this method as it has several drawbacks. You have to loosen the strings to a ridiculous degree to make room for the ferret to squeeze in there. You have to watch the ferret's diet so he doesn't get too fat for the job, and despite the fact that you rarely drop a pick in your guitar, the ferret must be fed daily. I haven't done the math, but it might be cheaper in the long run to just go with taking the back off.

This is what made me play with fingers! When I was just starting out I had 3 picks and they were all inside the guitar so I just kept playing.
