To me, it comes down to the "routine" the director/producer used in the final scene in the diner:

Tony is alone awaiting his family.

The bell on the front door chimes because someone has entered the diner.

Camera switches to looking at Tony so we can see him look up to see who's arrived.

Camera switches to Tony's perspective, and we see (as tony sees) who is entering.

This routine is repeated, exactly, every time.

When AJ arrives, there is a dude that walks in behind... Later in the scene, that dude goes to the restroom and walks by Tony.

Later in the scene, the entrance bell dings:

Camera looks at Tony, he looks up to see who's arrived. Camera switches to Tony's perspective, and it's black, because from Tony's perspective, all you see is black - because he's dead.

The dude that walked in behind AJ is a gangster there to kill Tony.
