Help needed with TGF Forum CD legal form

For those of you that have already been through this... I'm trying to get an entry for the forum cd out the door. Before I can send it, I need some help with this release form.
The very last part "In Witness Whereof"... what goes in each of these blanks (and tell me if it's supposed to be a signature or a printed name):

Entrant:
By:
Printed Name:
Date Executed:

And are there supposed to be 3 witnesses, 3 copies of this form, ??? what is the "Multiple (at least 3) counterparts" about???

Filling mine out too. Let me put my lawyer hat on for this one:
Entrant--name of the person submitting the CD
By--that's where your signature goes
Printed name--print your name below your signature

"Counterpart" means three duplicate originals (you sign each one) rather than original plus two copies.

I *think* that's what Gutch meant.

Thanks, Sandy. I got an email from Gutch earlier. He said the same about the fill-in-the-blanks stuff. I assumed 3 signed originals. That's what he's getting! Mine's going out today! :D
