How do you know when a neck needs a reset?
What do you look for to see if a guitar needs a neck reset? What are the symptoms? How old does a guitar usually get before a neck reset is needed?

If the top of the frets are in line with the bottom ( or well below the top ) of your bridge then ya need a neck reset. If they ( the top of the frets ) are in line with the top of your bridge you do not need a neck reset, unless of course the neck is warped or damaged.

If the bridge saddle has to be sanded down so far that there's little or none of it peeking over the bridge when viewed from the side, then you know you need a neck reset.

When there's nothing wrong with your saddle or your nut but yet you could still slide in a slice of pizza under your 12th fret.

I think your handle says it all - 12 string = neck reset - soon. I have two, one is wall art the other is on the way.

That's a top issue, not a neck reset issue.
